How We Validate Information

We obtain a practitioner's name, gender and office location on the credentialing application and validate it every two years as part of the re-credentialing process.

Board certifications, specialty, hospital affiliations, medical group affilitations and languages spoken are verified by the credentialing department through our every-other-year process. Practitioner name, office location, acceptance of new patients and specialty are verified yearly by the provider informatics department by contacting the provider offices for updates.

Facility name, location and accreditation is verified by the credentialing department upon initial credentialing and validated every two years as part of the re-credentialing process.
