Disease Management Programs

Network Health Plan (NHP) offers Disease Management programs for members with specific diseases or health conditions, including asthma, chronic obstructive pulmonary disease (COPD), chronic bronchitis, diabetes, heart failure, and heart/stroke.  Improved self-management can improve the daily quality of life for members with these conditions, so NHP offers communication with a Registered Nurse or other health professional to assist members with these conditions and to manage them more effectively.  This is a FREE service offered to you by NHP!

As a member, you may have already been enrolled in one or more of these programs through one of the following channels:

  • Physician referral
  • Hospital admission
  • Emergency room visit
  • Case manager
  • Insurance claim information
  • Utilization of healthcare services
  • Pharmacy information
  • Health Risk Assessment responses (Medicare Advantage members)
  • Self-referral
