Baby & Me

Steps to a Healthy Pregnancy

Pregnancy –What a unique and special time!

There is so much new information to learn about yourself and your baby. Network Health Plan is happy to offer you a program called "Baby & Me – Steps to a Healthy Pregnancy." It is designed to educate, inform and assist you during pregnancy and soon after delivery. All our "Baby & Me" staff are nurses with backgrounds in prenatal and postpartum care.

Care management coordinators are available if your baby is born prematurely or with special needs. Your "Baby & Me" nurse will continue to provide care advice and services. We will be in contact with you and your baby's doctor during your baby's hospital stay and when you and your baby are ready to go home. Our nurses will assist you in coordinating any home care services you may need.

To enroll in this program, contact Network Health Plan’s Care Management department at (920) 720-1600 or 1-800-236-0208 ext. 1600 or enroll through your health care provider’s office.

Your Pregnancy Is Important to Us

Step 1

A nurse from Network Health Plan Baby & Me will contact you around your 12th week of pregnancy to ask you a few questions about yourself and past pregnancies. Our goal is to determine any factor that may affect the outcome of your pregnancy. If we feel you would benefit from early monitoring, we will immediately refer you to care management coordinators. The coordinator will help you and your doctor arrange for any specialty care or services. Your coordinator will remain in contact with you throughout your entire pregnancy and help you address any questions or problems.

Step 2

If your pregnancy does not require special early monitoring, you will receive another call from a "Baby & Me" registered nurse around your 27th - 30th week of pregnancy. This call will include additional information, education and review of your pregnancy and upcoming delivery.

Step 3

After you deliver your baby and are discharged from the hospital, you will receive a final call from the "Baby & Me" nurse. We would like to discuss any questions or concerns you may have regarding yourself or your baby. We will send you information on infant immunizations regarding the best time to begin them as well as other printed material.
