Disease Management Program E-Enrollment Form
If you have been diagnosed with asthma, diabetes, or have experienced heart problems* or a stroke, you are encouraged to take advantage of our personal disease management program. After you submit the information below, one of our nurses will contact you within two business days to talk with you about enrolling in the disease management program and assisting you with managing your health status.
*the Network Health Plan disease management program is also available to Medicare Advantage members who have been diagnosed with Heart Failure
