Enrollment - Activity Period - Close of Activity Period
Each year your employer offers you a chance to review your health benefits. As a Millennium member you (and your spouse, if applicable) can choose to complete the three steps of participation. You must complete the three-step participation process by the end of your Open Enrollment period in order to fully participate in this program.
At the end of your open enrollment you (and your spouse, if applicable) have a year to complete your health activities listed in your Personal Plan for Health.
At the close of your activity period, you (and your spouse, if applicable) will complete a follow-up HRA and will be notified of the amount of your deductible credits for the following year’s plan. These are your healthy rewards.
If you are already healthy you can still reap the Millennium benefits by maintaining a healthy lifestyle.
