Preferred Drug List

The Network Health Plan Preferred Drug List contains the most commonly prescribed medications with information about their cost, co-payment tiers, and restrictions, if any.

Tips for Using the Preferred Drug List

  1. Use the Bookmark panel to find the chapter in which the prescription medication is located. For example, drugs used to treat heart conditions are listed in the “Cardiovascular Medications” category. If you know the medication’s category, click the category name listed in the Bookmark panel and then scroll through the category list to locate the medication.
  2. If you don’t know the medication’s category, the Index link at the bottom of the Bookmark list provides an alphabetical list of all medications in the document. Scroll through the Index to find the page number for the medication and then go to that page to locate the medication.
  3. The columns on each page list the relative cost of the medication, any restrictions that apply and the Tier in which the medication is classified.
  4. Abbreviations are explained on the bottom of each page. The tier numbers relate to the co-payment amounts required for the medication according to the participant's pharmacy benefit schedule.

Express Scripts Pharmacy Benefits

Most Network Health Plan includes the Express Scripts Pharmacy Benefits Manager with almost all it group health plans. The Express Scripts website is available to participants by signing in to their account and clicking on Express Scripts or My Prescriptions. Online Express Scripts services include:

  • Formulary Status - gives the formulary status of prescription medications and explains alternatives if the prescription medication is not on the Preferred Drug List
  • Personal Reminder Service - sends email reminders when it's time to reorder prescriptions
  • Pharmacy Locator - offers directions and maps to the nearby retail pharmacies
  • 12-Month Prescription History & Personalized Drug Information - displays a participant's prescription history and details
  • Mail Order Service - allows participants to order prescriptions, transfer prescriptions to mail order, and check the status of a current order

  • The information contained in the Preferred Drug List is not a guarantee of coverage. Co-payments and benefits are determined when the pharmacy claim is submitted to Express Scripts.

Prescription Drug Claim Form

When prescription claim can’t be processed at the pharmacy, a Prescription Drug Claim Form can be used to mail the claim to Express Scripts, Network Health Plan’s pharmacy benefits processor. The form can be completed by typing the information from a computer and then printing the form, or it can be printed and completed it by hand. Additional instructions about completing and submitting the form can be found on the second page of the form.

Express Scripts Exceptions

The following companies do not use Express Scripts for the processing of their pharmacy benefits. These employees should check with their employees for more information about their pharmacy benefits.
