Case Management Services
When your Network Health patients are diagnosed with a chronic health condition, we will contact them to offer our no-cost health management services, including condition-specific educational materials that empower them to become active participants in their own care. Our goal is to work with you to help them manage their health condition and symptoms through things like educational materials, classes, and self-care information. They will also have access to the services of our case management nurses and other health professionals, free of charge. Our nurse case managers will:
- Coordinate the care and attention they receive from their physicians. Being diagnosed with and living with a chronic condition can be overwhelming. Our nurse case managers work with you to make sure that the appropriate care is delivered for each patient’s specific situation and that they receive the services they need at the time they need them. We will work with you to ensure that the standards of care we deliver are based on scientific evidence, best practice guidelines and appropriate protocols so that your Network Health patients get care that fits their specific needs.
- Prevention of complications from chronic conditions. We want to support the strategies you will use to prevent complications from chronic conditions so that our participants can maintain and improve their health. We believe in self-management approaches and support all our providers in their efforts to deliver superior clinical outcomes and excellent customer service.
Living Well with Chronic Conditions
Trained facilitators lead Living Well with Chronic Conditions and Living Well with Diabetes classes throughout the year that are targeted to those who have been diagnosed with a chronic condition. The classes teach participants how to make lifestyle changes that will help them avoid complications from their disease and things they can do to feel better on a day-to-day basis.
