Care Management Services
When you are diagnosed with a chronic disease, Network Health Plan is here to help. We want to make sure you get your questions answered after your diagnosis, and so that you can manage your symptoms and maintain and improve your physical and mental well-being.
When you are diagnosed with a chronic condition, there are several ways we help:
- Coordinating the care and attention you receive from our contracted providers. Being diagnosed with and living with a chronic condition can be overwhelming. We will work with your providers to make sure that you get the appropriate care for your situation and that you receive the services you need at the time you need them. It’s important to us that standards of care you receive are based on scientific evidence and best practice guidelines and protocols so you and your family can be sure that your care fits your specific needs and situation.
- Prevention of complications from chronic conditions. We want to support the strategies that you and your provider will follow to prevent complications from your chronic condition so that you can maintain and improve your health. We believe in self-management approaches and support our providers in their efforts to deliver superior clinical outcomes and excellent customer service.
If you are a Network Health Plan participant and you are diagnosed with a chronic health condition, we will contact you to offer our no-cost health management services, including condition-specific educational materials that empower you become an active participant in your own care. Our goal is to help you manage your health condition and symptoms through things like educational materials, classes, and self-care information. You also have access to the services of our care management nurses and other health professionals, free of charge!
Living Well with Chronic Conditions
Trained facilitators lead Living Well with Chronic Conditions and Living Well with Diabetes classes throughout the year for those who have been diagnosed with a chronic condition. The classes teach participants how to make lifestyle changes that will help them avoid complications from their disease and things they can do to feel better on a day-to-day basis.
