Express Scripts Prescription Benefits
Sign in to My Account to access information about your pharmacy benefits, including:
- Formulary Status - gives you the status of your prescription medications and explains alternatives to consider if your prescription medication is not on the Preferred Drug List
- Personal Reminder Service - sends you an email when it's time to reorder your prescriptions
- Pharmacy Locator - gives you directions and a map to the retail pharmacy closest to your home or workplace
- 12-Month Prescription History & Personalized Drug Information - displays your prescription history and offers information about your prescriptions
- Mail Order Service - allows you to order prescriptions, transfer your prescriptions to mail order, and check the status of a current order
Preferred Drug List
The Network Health Plan Preferred Drug List contains the most commonly prescribed medications with information about their cost, co-payment tiers, and restrictions, if any. For more information, please visit the Express Scripts website or call Express Scripts at (800) 417-3380.
Tips for Using the Preferred Drug List
- Use the Bookmark panel to find the chapter in which your prescription medication is located. For example, drugs used to treat heart conditions are listed in the “Cardiovascular Medications” category. If you know your medication’s category, click the category name listed in the Bookmark panel and then scroll through the category list to locate your medication.
- If you don’t know your medication’s category, the Index link at the bottom of the Bookmark list provides an alphabetical list of all medications in the document. Scroll through the Index to find the page number for your medication and then go to that page to locate your medication.
- The columns on each page list the relative cost of the prescription, any restrictions that apply to the medication, and the Tier in which the medication is classified.
- Abbreviations are explained on the bottom of each page. The tier numbers relate to the co-payment amounts you will pay for the medication according to your employer's pharmacy benefit schedule.

Prescription Drug Claim Form
When your prescription claim can’t be processed at the pharmacy, you can download a Prescription Drug Claim Form and use it to mail your claim to Express Scripts, Network Health Plan’s pharmacy benefits processor. You can either complete the form by typing your information from your computer and then printing the form, or you can print the form and complete it by hand. Additional instructions about completing and submitting the form can be found on the second page of the form.
