Care Management Services

When Network Health Plan participants are diagnosed with a chronic health condition, we contact them to offer our no-cost health management services, including condition-specific educational materials that empower them to become active participants in their own care. Our goal is to help them manage their health condition and symptoms through things like educational materials, classes, and self-care information. They also have access to the services of our care management nurses and other health professionals, free of charge!

1. Coordinating the care and attention they receive from our contracted providers. Being diagnosed with and living with a chronic condition can be overwhelming. Our nurse case managers work with the providers to make sure that the appropriate care is delivered for each person’s specific situation and that they receive the services they need at the time they need them. It’s important to us that the standards of care followed by our providers are based on scientific evidence, best practice guidelines and appropriate protocols so that your employees get the care that fits their specific needs.
2. Prevention of complications from chronic conditions. We want to support the strategies that the provider will follow to prevent complications from chronic conditions so that participants can maintain and improve their health. We believe in self-management approaches and support our providers in their efforts to deliver superior clinical outcomes and excellent customer service.

Living Well with Chronic Conditions

Trained facilitators lead Living Well with Chronic Conditions and Living Well with Diabetes classes throughout the year for those who have been diagnosed with a chronic condition. The classes teach participants how to make lifestyle changes that will help them avoid complications from their disease and things they can do to feel better on a day-to-day basis.
